IARP Certificate Service Order Form

Reward your students with professional IARP Reiki certificates. All certificates are printed individually on 8 ½ x 11 quality certificate stock with professional engraver's lettering.
This is a service for IARP Members ONLY – you must be a current IARP member to take part in the Certificate Service Program.

To Order: Simply choose your certificate type, color preference, and wording below. Most teachers choose Standard Wording but if you wish to have special wording you may enter it below on form. Complete form and click Submit at bottom of form. You will then see a confirmation screen and Member Services will email you with payment options.

Certificate Types:
Standard Certificates: are available in packages of 25, 50, 75 or 100, include blank lines for student name and level of Reiki for you to write in on each certificate.
Custom Certificates: we include the students' name and level of Reiki.

IARP Certificate Service Program costs are:
• 25 Standard Certificates are $75 ($3 each)
• 50 Standard Certificates are $137.50 ($2.75 each)
• 75 Standard Certificates are $195 ($2.60 each)
• 100 Standard Certificates are $248 ($2.48 each)
• Custom Certificates are $4.75 each, no minimum
Shipping is $6.95 per order within the USA, Canada and Mexico and US$12.95 for most other countries.

Certificate Heading Field:
Type in your desired heading that will appear at the top of your certificates such as Certificate of Reiki or Usui Shiki Ryoho or Certificate of Achievement or your center or business name. *
Maximum Allowed: 50 characters. Currently Used: 0 characters.
Students' Names and Reiki Degree Field:
-If ordering Standard Certificate (25, 50, 75 or 100) leave this next field blank. Or if you are ordering individual certificates and would like to write in your students' names and levels of Reiki yourself then leave this next field blank and we will include blank lines for student name and level of Reiki.

-If you are ordering individual Custom Certificates and want us to complete the students' names and level of Reiki then type them below. Example: Susan Smith - Reiki 1, John Jones - Reiki 2, Jim King - Reiki 1, Jim King - Reiki 2.
Type of Reiki (and/or Special Wording) Field:
- List the type of Reiki taught such as Usui Shiki Ryoho, Usui/Tibetan Reiki, Usui Reiki Ryoho, etc. in the field below. Your certificates will be printed with our Standard Sample Wording as shown below unless you specify otherwise. If you want special wording please enter entire wording desired (to appear after student name) in the below box. Otherwise just type in the type of Reiki and we will include the standard wording as shown.
